What Kent is doing to tackle health inequalities

Making sure the Direct Enhanced Scheme to offer annual health checks is working well across the county. A GP trainee has been working at a practice level to see what difference health checks are making to individuals.

Communication folders have been introduced to all wards and departments in acute, community hospitals and now to mental health wards.

Health Diaries – over 500 health diaries have been given to people with learning disabilities across the county to record their experiences of health services.

Auditing Health action Plans – we are trying to find out if people have health action plans and what difference they have made to people’s lives.
