Main areas of work 2010-2011: We aim to have up-to-date registers in all GP practices of people with learning disabilities and family carers, with a data sharing agreement in place across the PCT area.

2.2. Primary Care Teams tackling health inequalities

Progress notes: An easy read information leaflet about annual health checks was produced by the Primary Health Care Facilitator, working with self advocates, a local GP and PCT Commissioners. These leaflets were distributed by the local authority to people they identified as having a learning disability
