Progress notes: 100% practices with QOF Registers. All validated 09-10. Differing GP systems hampers electronic data collection. Despite difficulty of having range of systems, have given comprehensive info on all points

2.2. Primary Care Teams tackling health inequalities

Progress notes: Positive feedback from people about GPs and primary care staff. Easy read leaflets produced for GPs to send out about health checks. For more info contact: TBC. Training needs of p/care staff identified and being addressed. Increase in DES take up (88%) and progress made on AHCs. Detailed notes from Big Health Check event provided, show people feel there is positive progress. 2.3. Equal access to disease prevention, screening, and health promoting activities

Progress notes: Good initiatives described include development of service provider 'Health Champions' where five Health Champions have been trained through "Healthy Halifax" initiative, to support people with learning disabilities to attend a variety of healthy initiatives including, aerobics sessions in the local community, healthy eating initiatives, chair aerobics etc. For more info contact: TBC. Little data given on access to specific screening programmes. Roll out of AHCs and later DES audit should help - however, lack of data is still a key deficit in terms of being able to gauge current health status - and then, later, comparative health outcomes

2.4. Services in wider primary care community

Progress notes: Plans to improve discharge for people with long term conditions, and build links with community matrons. Targeted work being done with mainstream providers includes: - Falls prevention team provide support to people with learning disabilities. - Evaluation of eye care services included people with learning disabilities. - Training is available for supporting good oral hygiene for people with learning disabilities. - Additional Oral Health questions developed for GPs to include in Annual Health Checks. 2.5. Service Agreements secure a range of treatment choices and equity of access

Progress notes: All NHS Providers have self assessed against CQC standards set out in Access to healthcare for people with learning disabilities, and now have action plans. Range of initiatives in acute hospital, including offering telephone pre-assessments; development of 'champions' (55 in CHFT). For more info contact: TBC. Reviewer's note: Commend thorough approach to raising awareness and giving support in a range of ways across all mainstream providers- in particular work in acute hospital. 2.6. National Service Frameworks and linked networks and projects apply equally to people with disabilities

Progress notes: Response records positive joint working in range of areas (see also 2.4). Good focus on helping people understand issue of health inequality. Progress notes: Good progress made, in particular in acute hospital setting. VIP alert system planned last year is now in place on hospital PAS system, making possible reasonable adjustments on individual basis. Will also go on the A+E and radiology systems. Progress notes: Reviewer's note: Both HFA plan and now the SAAP suggest strong and coherent Partnership work across range of leaders in all organisations.

Progress notes: Response reports over 10 Best Interest Decisions completed by CTLD - and that these have supported people in securing positive health outcomes. Action planned here responds directly to feedback from carers and others.

Main areas of work 2010-2011: We will provide a range of information and training for parents and carers as well as health staff on understanding consent by December 2010

3.3. Review of complaints and adverse incidents leads to improved practice in services

Progress notes: SAAP shows detail of planned activity across a range of disciplines to learn from complaints and SUIs and to improve practice accordingly. Review and discussion of SUIs and complaints is built into contract review meetings. The local authority has appointed a contract compliance officer. with a learning disability - this officer will help support the team and give greater insight into the quality of service delivery.

4.03. Plans for local specialist services for young people in transition

Progress notes: Transition Strategy group for people aged 14-25 in place. Person centred Health Action planning pilot starts with year 9 pupils; 20 staff across the Children's Trust trained to facilitate reviews in special and mainstream schools. Specific worker employed to support parents with learning disabilities who are parents.

4.04. People and families well involved in planning and delivery of health services

Progress notes: Info provided shows well-attended events to talk about health: "It's my Health Day" and Big Health Check Day. Voice and Choices in January got wider range of people involved in telling the Partnership Board what is important to them in life generally. Self advocates are involved in delivering the training to primary care staff. Two people with learning disabilities were part of the panel to agree the approved provider lists for day services, homecare and supported living.

Progress notes: Joint Programme across NHS Calderdale and Council will shape commissioning and workforce planning. Will plan to move away from block contracts to more individualized contracts. New Contract agreed with provider including improved monitoring. New service specification being completed for Community Nurses (due June 2010)

4.09. Mental health policy and best practice equally applied to people with learning disabilities

Progress notes: People have access to IAPT, and protocols are in place for 24 hour access to crisis team. LD and Green Light included in MH contract review. Reviewer's note: Better mainstream MH access was a priority for Calderdale last year and work has clearly progressed well.

Progress notes: No prison in Calderdale and no known people from Calderdale currently in prison. Response records CTLD have good links with Criminal Justice system. Learning Disability awareness training given to Probation. Service providers commissioned to provide extra support to rehabilitate and reduce reoffending behaviour.
