Valuing people nowProgress notes: Derbyshire have developed a transitions care pathway (a plan that lots of people agree to and says what should happen, when and by who) for children coming into adult services. A Health Needs Assessment about people with learning disability who have an autistic spectrum condition has been done. Staff have been trained to diagnose autistic spectrum conditions so that people don't have to travel out of Derbyshire for this. |
